Information on this website is provided for general educational purposes and does not constitute medical advice, diagnosis, or treatment. Using this website or submitting a form does not create a provider-patient relationship. That relationship begins only when you are formally established as a patient.

Appointments & Scheduling
Requests submitted through this website are requests only and are not confirmed until you receive confirmation from our team. Appointment availability, provider assignment, and visit type are subject to clinical judgment.
Our providers are licensed to see patients in specific states. Each provider is listed on the booking page under the state in which they see patients, and care can only be provided where the treating provider is licensed.
Appointments are reserved specifically for you. If you need to reschedule or cancel, please give us at least 24 hours’ notice. A $50 fee applies to missed appointments and to cancellations made with less than 24 hours’ notice. You will be asked to agree to this policy at the time of booking.
Memberships & Billing
Coastal operates on a membership model rather than through insurance billing. Membership pricing, inclusions, and exclusions are described on the relevant service page and at the time of enrollment. Membership fees are not insurance and do not cover services provided by outside laboratories, pharmacies, specialists, or facilities.
No products or services are sold directly through this website. All invoicing is managed securely through your patient account in Charm Health, our Electronic Health Record and patient portal partner.
SMS & Messaging
SMS consent communication. Mobile opt-in, SMS consent, and phone numbers collected for SMS communication purposes will not be shared with any third party or affiliates for marketing purposes.
Types of SMS communications. If you have consented to receive text messages from Coastal Primary Care and Wellness, you may receive messages related to the following:
- Appointment reminders
- Follow-up messages
- Billing inquiries
- Promotions or offers, where applicable
Example: “Hello, this is a friendly reminder of your upcoming appointment with [Provider Name] on [Date] at [Time]. You can reply STOP to opt out of SMS messaging from Coastal Primary Care and Wellness at any time.”
Message frequency. Message frequency may vary. You may receive up to 2 SMS messages per week regarding your appointments or account status.
Potential fees for SMS messaging. Standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.
Opt-in method. You may opt in to receive SMS messages from Coastal Primary Care and Wellness in the following ways:
- Verbally, during a conversation
- By submitting an online form
- By filling out a paper form
Opt-out method. You can opt out of receiving SMS messages at any time. Reply STOP to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.
Help. If you are experiencing any issues, reply with the keyword HELP. You can also get help directly from us at info@coastalprimarycare.com.
Additional options. If you do not wish to receive SMS messages, you can choose not to check the SMS consent box on our forms.
